Your Support at Home budget can be used for a wide range of services to help you stay safe, well and connected at home. We’ll help you choose the right mix for your situation.

What Support at Home Means for You

Support at Home offers a mix of clinical care, daily assistance, independence supports and culturally safe services that help you stay well, feel confident and continue living life the way you choose.
Personal Care Assistance
Medication reminders, getting in and out of bed, dressing, showering, help using the toilet, shaving, etc.
Household Cleaning
Help with mopping, vacuuming, washing dishes, shopping, making the bed, laundry, ironing, etc.
Gardening Services
Minor garden maintenance like mowing the lawn, trimming hedges, weeding, pruning, watering, etc.
Aged Care Transport
Help with adovcating and transport you to appointments, shopping, social gatherings, outings, etc.
Social Outings
Company to share activities like having tea together, playing board games, outings, having a conversation, etc.
Allied Health Care
Includes clinical interventions, expertise, care and treatment, wound care and management, etc.
Support at Home Meals
Support to prepare meals in the home, meal delivery, advice on nutrition, hydration, etc.
In-Home Nursing
A registered nurse heling you at home for a short period of time.
Home Maintenance
Essential minor repairs and modifications to maintain and improve safety.

Additional Short Term Funding Pathways

Alongside your regular care funding, there are also three short term pathways that can offer extra support when you need it.

1. Assistive Technology and Home Modifications Scheme

Through AT-HM Scheme, you'll have access to the equipment and home changes you need to stay safe and independent, without using your regular care budget. Based on your assessment, It can cover things like mobility aids, bathroom equipment or home modifications.

2. Restorative Care Pathway

The Restorative Care Pathway gives you short term funding for focused support from allied health or nursing professionals. It’s designed to help you regain strength, improve day to day functioning and prevent further decline, all with a clear set of goals tailored to you.

3. End-of-Life Pathway

The End-of-Life Pathway offers short term support for older people who have been given a limited time to live and want to spend that time at home.
